How do I report an absence for my student?
Fill out the Attendance Form, and our Attendance Secretary will update student records.
How do I change my address?
Fill out the HOUSING QUESTIONNAIRE and return it via email to our Pupil Accounting Secretary at firstname.lastname@example.org, along with two current proofs of address, such as lease, deed, driver's license, utility bill etc.
How do I update my email address?
Log into your NYC Schools account at mystudent.nyc. If you do not have an account, fill out our Tech Support Request form to get a sign up code. Also, update your email in the PTA connect database by logging into your PTA connect account.
How do I update my phone number?
How do I update my emergency contacts on my child's Blue Card?
How can I request a Metrocard (or replace a lost Metrocard) for my child?
If a student loses their Metrocard, they can request a replacement by visiting the Main Office and entering their name into the logbook. They can pick it up the next school day. Read the Student Metrocard Overview.
How can I request a Letter of Verification for enrollment, housing, IRS, etc?
Please fill out this Google form. After two business days, your student can pick up the letter in the Main Office.
How do I find out if my child can receive financial assistance?
Fill out the Family Income Inquiry form online. This form was previously known as the "the lunch form". The school will receive a code indicating whether the student qualifies for financial assistance. Parent/guardian financial information is not shared with school staff. If the student is eligible eligible, they may qualify for fee waivers for standardized exams such as AP, SAT and ACT, fee waivers for field trips, and other assistance.
How do I get technical support?
If you are having trouble logging into online accounts or with school-owned devices, please fill out our Tech Support Request form, and a staff member will be in touch with you.
How do I request a device for remote learning?

Please ask your doctor to fill out the general MAF 2021-22.pdf , and email it to the Pupil Accounting Secretary at email@example.com. To upload COVID Vaccination information, visit the Vaccination Portal.
How does my child find out their DOE Student Account information?
A staff member can help a student reset or access their DOE Student Account information. DOE student account end in @nycstudents.net. Students can alert their teacher or visit Room 248 to have the account reset.

How do I request a new student ID?
Please fill out the Student ID Replacement Request Form. After two days, visit the Main Office to pick it up.
How do I request yellow bus service?
Only students in K-6 residing below 93rd st in Manhattan are eligible for public bus service. Fill out the Bus Service request and our Transportation Secretary will contact you. For private bus service, visit the PTA Transportation webpage.

How do I request Working Papers?
Please visit the Working Papers webpage to find the application forms and process for applying for Working Papers. One of the forms will need to be filled out by your doctor, and you will need a signed letter as well.
